Immunization

The University of South Carolina requires all students born after December 31, 1956 to be immunized against or provide proof of immunity to MMR (Measles, Mumps, Rubella). Both degree-seeking and non-degree seeking graduate students must enter their immunizations in the “Medical Clearances” portion of  sc.edu/myhealthspace  and upload supporting documentation providing proof of immunizations signed by a medical provider. (Note: You can take a photo of your documentation with your phone and upload the photo).

Fully online students must complete the Distance Learner Exemption form found under “Downloadable Forms” at sc.edu/myhealthspace each term prior to registration or upload your immunization record to avoid having to submit the exemption form each term.

Student Health Services is located in the Center for Health and Well-Being, 1401 Devine Street, behind the Russell House.

  • Submission deadlines are July 15 for Fall admission and December 1 for Spring admission.
  • For more information, call the Immunization Office at 803-777-9511.
  • For more information about required immunizations, visit the SHS Immunizations page
  • Immunization information is now submitted through sc/edu/myhealthspace
